“Pursuant to Law no. 18,331 of August 11th, 2008 on the Protection of Personal Data and Habeas Data Writ (LPDP, for its acronym in Spanish), the data you provide shall be incorporated into a database, which shall be treated exclusively for the following purpose: **Purpose of the form**.
